- PharmD degree from an ACPE-accredited college of pharmacy
- Eligibility for Illinois licensure
- Completion of an ASHP-accredited PGY-1 pharmacy residency
- Letter of intent

- Three (3) letters of recommendation (two (2) from clinical preceptors preferred)

- On-site or virtual interview
